“Nootropic” has become one of the more elastic terms in the supplement industry. Technically, it refers to substances that enhance cognitive function — memory, focus, processing speed, mental clarity — without significant side effects. In practice, it’s applied to everything from well-studied compounds with decades of research to proprietary blends where you can’t determine the dose of any individual ingredient.
This review focuses on the most common nootropic stack pattern — caffeine plus L-theanine — and then examines what the broader “stack” trend actually delivers.
The one combination with real evidence: caffeine + L-theanine
If there’s a nootropic combination that earns its claims, it’s caffeine paired with L-theanine. This isn’t speculation — it’s one of the more thoroughly studied combinations in the cognitive enhancement literature.
Caffeine alone improves alertness, reaction time, and sustained attention. This is not controversial. The mechanism is well understood: adenosine receptor antagonism reduces the subjective feeling of fatigue. The typical ratio studied is 2:1 L-theanine to caffeine (e.g., 200mg L-theanine + 100mg caffeine).
This combination has meaningful evidence. If a nootropic stack is built around it at these doses, there’s something to it.
What the evidence says about other common nootropic ingredients
Lion’s Mane mushroom (Hericium erinaceus): Proposed to stimulate nerve growth factor (NGF) production. There is one notable small Japanese RCT showing improvement in mild cognitive impairment over 16 weeks. Human trial data beyond this is sparse. Promising, but very early.
Bacopa monnieri: One of the better-studied herbal nootropics. Several RCTs have found improvements in memory acquisition and retention — but effects are slow, typically requiring 8–12 weeks of daily use before becoming apparent. Not a productivity supplement; more of a long-term memory support option.
Rhodiola rosea: An adaptogen with some evidence for reducing mental fatigue under stressful conditions. Effects on raw cognitive performance in healthy, non-fatigued individuals are less clear.
Phosphatidylserine: A phospholipid with a reasonable evidence base for cognitive decline in older adults. Evidence for cognitive enhancement in healthy younger adults is thin.
Citicoline (CDP-choline): Reasonably good safety profile, some evidence for cognitive support in people with impaired cognition. Evidence for enhancement in healthy individuals is limited.
The “proprietary blend” problem
Many commercial nootropic stacks use proprietary blends — a listed set of ingredients with a total blend weight but no individual ingredient doses. This makes it impossible to know whether any ingredient is present at a clinically meaningful dose.
A product might list “Lion’s Mane, Bacopa, Rhodiola, Phosphatidylserine, Citicoline — 400mg proprietary blend.” With five ingredients sharing 400mg, the math doesn’t work for any of them to be at a dose that’s shown effects in trials. You’re paying for a label, not a formulation.
This isn’t always the case — some brands publish full ingredient doses — but it’s common enough that label reading is essential before buying any nootropic stack.
The subjective experience problem
Nootropics are uniquely susceptible to placebo effects and expectancy bias. Cognitive performance is highly subjective. If you take something that tastes medicinal, smells earthy, and cost $80, and you’ve read testimonials about it improving focus, you are primed to notice moments of good focus and attribute them to the supplement.
This isn’t a dismissal — placebo effects are real and sometimes useful. But it does mean that “I feel sharper” is not evidence that a supplement is working at a pharmacological level.

Least likely to see benefit: People who are already well-rested, well-nourished, and cognitively performing at their baseline. The research literature consistently shows that cognitive enhancement supplements produce the largest effects in people who are impaired relative to their normal function — not in people who are already optimised.
Sleep, nutrition, and exercise remain the highest-leverage interventions for cognitive performance. A nootropic stack on top of a sleep-deprived, sedentary baseline is a poor investment.
